First Visit Summary A Quick Overview Once you have filled out all necessary forms, the provider you are scheduled with will ask a few questions pertaining to your condition as well as current & past health history. Following the history, an examination will be performed according to the patient and presenting complaint. You will receive treatment on the first visit following the examination and future treatment frequency and protocol will be discussed.

Yes you will need to complete a patient intake form and a consent form. These forms take about 10 minutes to fill out, so we recommend downloading the FORMS and completing them at home prior to your first visit. Otherwise the forms can be completed in our office 10 minutes prior to your scheduled appointment time. It is important that you either bring the completed forms with you or arrive early so that your appointment can begin at the scheduled time.

Depending on which provider you are scheduled with determines the amount of time allotted for your appointment. First visits with Dr. Friedline typically take 45 minutes and first visits with a therapist will be 30 minutes. Follow up visits will vary according to the type of treatment you will receive and the number of healthcare providers you see. Chiropractic follow up visits usually take about 15-30 minutes and therapy follow up visits are scheduled in 15 minute intervals up to 1 hour.
Insurance Yes we do accept insurance. We are in the process of contracting with many of the major healthcare companies, however, until that time many insurance policies have out-of-network benefits that can be utilized for chiropractic and related therapies. We can call to check on your coverage to see if you have chiropractic and physical medicine coverage on your policy. A copy of your insurance card will be needed to verify benefits, and then benefits are only guaranteed after claims have been made and an explanation of benefits is returned to us. If you would like to have our office check on the coverage of your insurance policy, please give us a call at (970)-663-6501. Payment Payment is expected at the time service is rendered unless otherwise discussed. If you choose to use your insurance to pay for the services rendered, the co-pay / co-insurance will be due at the time of service. If you have any questions or concerns about ability to pay please let us know, we wish to make healthcare affordable to all and will explore alternative payment plans if necessary.
Consultations Consultations are available to answer any questions you might have about the services provided at our clinic or healthcare questions in general. These consultations are provided free of charge, but must be scheduled for a 20 minute appointment time. If you wish to schedule a consultation please contact Susan at (970)-663-6501. Health History At Center for Advanced Bodywork we believe in treating the whole person and therefore we want to know about all the conditions or symptoms that you currently have and any past history of conditions or symptoms. The provider you are seeing will ask you a number of questions regarding the information you put down on the intake form to get a better understanding of your body as a whole and any complicating factors that may be contributing to your pain or injury. Examination The examination will consist of functional as well as structural assessment in order for our providers to get an understanding of what is happening with you body as a whole. Our examinations typically include orthopedic, reflex, and motor evaluations, as well as a visual and hands on inspection of the area of concern. Our intention is to treat your body as a whole, not just as a symptom. Treatment Treatment will be rendered on the first visit in almost all cases, unless contraindicated. The treatment received will correlate to the presenting complaint and the findings in the health history and the examination. Treatment will consist of manual therapy, massage therapy, and/or chiropractic care, depending on which therapist or provider is seen and the injury or complaint presented. The first treatment is included in the first visit/exam fee. Each patient is different and the frequency and number of visits will vary between individual patients, depending on the injury and any complicating factors. We regularly get 80-100% improvement in as little as 6-8 visits. We know the human body is a dynamic organism and therefore our treatment schedule and protocols are dynamic as well to adapt to how your body responds to the treatment and therapies applied to it.
