Sports Physicals

Call (970)-663-6501 to schedule your physical before practice starts. In the state of Colorado sports physicals are required for participation in interscholastic athletics.

You can download the CHSAA Parent Athletic Permission Form and the Preparticipation Physical form by clicking on the image above. Or you can download it from the CHSAA website here.

There are two pages, the first page is the Parent/Student Permission form and the second page is the Physical, please bring the Physical form to your scheduled appointment.

If the athlete is under the age of 18, please complete the Minor Consent form. This form must be signed by a parent/guardian before a preparticipation physical can be performed on anyone under 18 years of age.

According to CHSAA by-laws:

No pupil shall participate in formal practice or represent his/her school in interscholastic athletics until there is a statement on file with the principal or athletic director signed by his/her parents or legal guardian and a practitioner licensed in the United States to perform sports physicals certifying that: (a) he/she has passed an adequate physical examination within the past 365 calendar days; (b) that in the opinion of the examining licensed practitioner, he/she is physically fit to participate in high school athletics; and (c) that he/she has the consent of his/her parents or legal guardian to participate.

Once you have had a physical it is good for 365 days, so you don't need to get one for every season, just once a year.
